Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service plans and implement them.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and accessing community resources.
• Implement activities that promote community inclusion and meaningful relationships.
• Maintain a safe, clean, and supportive environment for individuals served.
• Review and maintain required documentation accurately and timely.
