Open Sky Community Services in Leominster, MA is seeking a Residential Counselor to provide direct care, guidance, and skill-building support in residential and community-based settings.

You will assist with daily living activities, help individuals pursue personal goals, navigate transportation and community resources, and document progress in a safe, respectful, trauma-informed environment.

Formación

• High school diploma or equivalent is required.
• Valid driver’s license and acceptable driving record required.
• Reliable, willing to learn, and open to new opportunities.

Responsabilidades

• Assist in developing individualized, person-centered service and support plans and play an active role in their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and the development of valued social roles.
• Maintain a safe, clean, and supportive environment that promotes health, safety, and well-being.
• Review, complete, and maintain all required documentation in an accurate and timely manner.
