Axiom Care in Phoenix, AZ is seeking a licensed behavioral health clinician to provide individual, family and group counseling with a trauma-informed approach. You will coordinate care with stakeholders and maintain comprehensive EMR-based records to support client progress and outcomes.

The role emphasizes collaboration with the treatment team, adherence to CLAS and HIPAA standards, and ongoing professional development. A Master’s degree and AZ licensing are highly valued.

Qualifications

• Master’s degree in psychology, social work or counseling required.
• Licensed with AZBBHE preferred.
• 2-3 years in behavioral health/substance abuse preferred.
• Proficient with EMR and Microsoft Office.
• Fingerprint Clearance Card upon hire.
• First Aid/CPR cert within 90 days of hire.

Responsibilities

• Provide individual, family and group counseling with trauma-informed care.
• Coordinate care with stakeholders and funding sources.
• Maintain clinical documentation in EMR.
• Oversee accuracy of client records and care plans.
• Collaborate on individualized service plans and team meetings.
• Refer and assess level of care for clients.
