Job summary

CMJ RECOVERY AZ LLC in Phoenix, AZ is seeking a Clinical Therapist to provide individual, group, and family therapy within our Veterans Program. You will report to the Clinical Director and maintain HIPAA-compliant records.

The role includes crisis intervention, biopsychosocial assessments within 72 hours of admission, treatment planning within 72 hours and ongoing updates every 30 days, plus chart management in the KIPU system.

Qualifications

• Master’s degree in a related field is required.
• Licensure in LADC/ACSW/LCSW/APCC/LPCC/AMFT/LMFT is required.
• Proficient in HIPAA and confidentiality standards.

Responsibilities

• Provide individual, group, and family therapy.
• Conduct crisis intervention when needed.
• Perform and document biopsychosocial assessments within 72 hours of client admit.
• Collaborate on treatment plans and complete within 72 hours of admit.
• Update treatment plans at least every 30 days.
• Maintain accurate client charts and electronic documentation in KIPU system.
• Participate in mandated trainings and team meetings.
