Job summary

Little City Foundation is seeking a qualified QIDP to initiate and perform a range of casework for children in residential settings on the Palatine Campus and in the community.

The role requires a college degree meeting federal standards for QIDP and at least one year of experience with individuals with developmental disabilities, plus a valid IL driver’s license and strong communication skills.

Qualifications

• Must have a college or university degree meeting federal standards to serve as QIDP and at least 1 year experience with individuals with developmental disabilities.
• Must be at least 21 years of age.
• Excellent verbal and written communication and organizational skills.
• Must have a current IL driver’s license.

Responsibilities

• Carries a caseload of children in residential homes and community.
• Conducts weekly visits to ensure services are implemented and collaborate with staff.
• Secures documentation, reviews history, and makes recommendations.
• Collaborates with interdisciplinary team on Mental Health Assessments and ITPs.
• Attends IEPs and advocates for children in school.
• Monitors placements, ensures permanency and discharge planning.
• Maintains child files and coordinates with DCFS and DHS.
