Job summary

Habilitative Systems, Inc. (HSI) is seeking a QIDP/Case Manager in Chicago, IL to ensure services align with the individual service plan and agency policies. Regular full-time position supporting clients with developmental disabilities and related human service needs.

The role requires collaboration with counselors, instructors, families, and external professionals to maximize client outcomes and comply with funder and licensing requirements.

Qualifications

• Bachelor’s degree in a related field
• One (1) year QIDP experience required
• Current CPR certification required

Responsibilities

• Work cooperatively with other department counselors and instructors, referral/sources, parents, guardians to maximize client services.
• Document the Individual Service Plan (ISP) for individuals receiving services made by medical and professional consultants.
• Communicate with parents/guardians, peers, staff, and other community members regularly to maintain positive relationships and to ensure that service and support are implemented and continue to meet the individuals desired outcomes and needs.
• Seek opportunities for community engagement and applicable external services based on client’s individual needs and in accordance to the individual service plan.

Education

Bachelor’s Degree in special education rehabilitation or a related human service field

SUMMARY

Ensures that clients receive the services outlined in the Individual Habilitative Systems service plan in accordance with the policies of the agency, funder, licensing and accreditation entities.

QUALIFICATIONS

• Bachelor’s Degree in special education rehabilitation or a related human service field from an accredited college.
• One (1) year QIDP experience required.
• CurrentCPR certification.
