Job summary

Little City Foundation in Palatine, IL seeks a qualified caseworker to support children in the residential program. You will manage a caseload, conduct regular visits, document progress, and collaborate with staff to ensure appropriate services and treatment plans.

Responsibilities include attending IEPs, developing and updating Individual Treatment Plans, coordinating with DCFS and school personnel, and supporting family engagement while adhering to licensing standards and safety protocols.

Qualifications

• Must have a college degree meeting federal standards to serve as a QIDP.
• At least one year of direct experience with individuals with developmental disabilities.
• Must be at least 21 years old and have a valid IL driver’s license.

Purpose

To initiate, facilitate, and perform a variety of casework related functions pertaining to children residing in the children’s residential program.

Essential Duties and Responsibilities

• 1. Carries a caseload of children living in assigned homes, who reside in the residential homes on Palatine Campus and in the community.
• 2. Conducts at least weekly visits with each assigned child in his/her residential home to ensure appropriate services are implemented for the child as well as collaborate with all staff related to the child’s overall care, progress and treatment.
• 3. Secures documentation, compile historical information, assesses case situations and makes recommendations regarding children on caseload.
• 4. Collaborates with the interdisciplinary team in developing and implementing the Mental Health Assessment and Individual Treatment Plans for all cases, including quarterly and six-month reports.
• 5. Attends IEPs and advocates for children in the school system.
• 6. Attends monthly team meetings for the homes assigned.
• 7. Monitors and supervises the placements and adjustments of children. Responsible for permanency planning and discharge planning for children.
• 8. Attempts and maintains regular, ongoing contacts with the parents/guardians of children. For DCFS wards, makes all efforts to establish a relationship with the biological family and assists the biological family in reaching reunification when applicable to the case.
• 9. Responsible for the upkeep of child’s files which includes obtaining consents, monitoring and completing progress notes, and securing all required documentation for assessment and provision of services to child.
• 10. When applicable, works in conjunction with the DCFS worker, for implementation and coordination of sibling and birth parents’ visits.
• 11. Assures that licensing standards and applicable laws, rules, and policies are followed.
• 12. Assists with the intake process of new children moving into assigned homes, including setting up the child’s file and following up to ensure we receive all necessary documentation.
• 13. Work cooperatively with other staff in Children’s Services, DCFS, DHS and collateral resources.
• 14. Responsible for the facilitation of 6-month staffings, as well as, attendance in staff meetings, committee meetings supervision and trainings as required.
• 15. Responsible for all safety activities in your workplace and immediately report all accidents and safety hazards to your supervisor.
• 16. Successfully completes Little City Foundation required training, re-training, and any additional training as required by Little City Foundation policies and practices, or as assigned by his or her immediate supervisor.
• 17. Other duties as assigned by your supervisor.

Qualifications

MINIMUM QUALIFICATIONS:Must have a college or university degree that meets the federal educational standards required to serve or to be eligible to serve as a Qualified Intellectual Disability Professional (QIDP) and have at least one-year experience working directly with individuals with developmental disabilities, required. Must be at least 21 years of age. Must have excellent verbal and written communication skills as well as organizational skills. Must be capable of transporting children and possess a current IL valid driver’s license.

Physical Requirements & Work Environment

While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and lift up to 50 pounds with varying amounts of assistance on a reoccurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Occupational exposure to blood, body fluids, infectious substances, including communicable diseases when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants. Residential work environment with a noise level of normal to loud.
