Job summary

Holcomb Behavioral Health Systems - A Chimes Company in Kennett Square, PA is seeking a Residential Caseworker who helps residents reach self-sufficiency through individualized interventions and supportive treatments. The role emphasizes Recovery Model principles, life-skills coaching, crisis intervention, and collaboration with treatment teams.

Requires Associate's Degree and at least 1 year in health or human services, plus valid driver's license.

Qualifications

• Associate's Degree with at least 1 year of experience in a health & human service-related field (preferred).
• CNA experience and recent graduates are strongly encouraged to apply.
• Clearances: Criminal Clearance and Fingerprint-based federal criminal history.
• Valid Driver’s License (required).

Responsibilities

• Provide person-centered services in accordance with Recovery Model principles and program standards.
• Assist residents with life skills development, including budgeting, self-care, medication management, and community living skills.
• Provide supportive counseling and crisis intervention when needed.
• Develop and implement individualized service plans.
• Connect residents with community resources and support services.
• Maintain accurate and timely documentation
• Collaborate with treatment teams, providers, and supports to promote positive outcomes.

Job Description

In this role, the Residential Caseworker will help each consumer reach a maximum level of self-sufficiency through individualized interventions as well as the support of an overall therapeutic milieu. Interventions are based on service plans developed collaboratively with individual consumers.
