Chimes is seeking a Case Manager to deliver community-based behavioral health services in Sussex County, Delaware. You will assess client needs, monitor resources, and help individuals live safely in the community while managing a caseload of up to 30 clients.
The role requires crisis intervention, recovery-model-based practice, and collaboration with families and service providers. Monday–Friday 9:00 am–5:00 pm with on-call rotations, offering competitive pay and full-time benefits.
