Qualifications

• Bachelor’s degree in human services or related field required.
• Minimum 2 years experience in Human Service field; disability experience preferred.
• Valid driver's license and auto insurance required.

Responsibilities

• Provide assessment, referral, education, treatment, and follow-up services to clients on mental health, medical, substance abuse, vocational, and living skills.
• Collaborate with providers, agencies, and individuals in the client’s network of care.
• Monitor psychiatric signs, symptoms, and medication regimen; document findings.
• Provide crisis intervention and accompany client to appointments; advocacy as needed.
• Develop and implement person-centered recovery plans; maintain client records and billing.
