Job summary

Project Transition seeks a Residential Advisor to support Members in residential settings in Mohnton, PA. This full-time role covers 2nd and 3rd shifts with responsibilities for safety, medication support, and Daily Living activities.

You will document progress, assist with group sessions, and ensure adherence to the treatment plan. A high school diploma and a valid driver's license are required; compassionate, non-judgmental care is essential.

Qualifications

• High School Diploma or equivalent.
• Valid, non-commercial driver''s license with clean record.
• Ability to maintain confidentiality.

Responsibilities

• Oversee residential living arrangements to ensure safety, cleanliness and recovery environment.
• Document and support self-administration of medications.
• Assist with activities of daily living and transportation as needed.
• Complete incident reports and progress notes in EHR within required timelines.
• Co-facilitate groups and participate in supervision and program meetings.
