Job summary

Project Transition in Sinking Springs, PA, seeks a compassionate caregiver to oversee Member residential living environments and support daily living activities, including medication self-administration and safety protocols.

You will perform roving inspections, document care in the EHR, assist with transportation, meal planning, and group sessions, and collaborate with clinicians to ensure high-quality, person-centered care.

Qualifications

• High School Diploma or equivalent is required.
• Ability to maintain confidentiality.
• Valid, non-commercial driver's license with clean driving record.

Responsibilities

• Oversee resident living arrangements for safety and cleanliness.
• Document Member self-administration of medications.
• Perform roving inspections and ADL support as needed.
• Complete Incident Reports and document in EHR (DAP format).
• Facilitate groups and document within 24 hours of sessions.
• Assist with transportation and meal planning.
