Job summary

The Salvation Army in Glendale, NY seeks a residential support worker to provide a safe, clean, home-like environment and coordinate with healthcare staff. You will help residents with daily living activities and monitor medical appointments.

Responsibilities include maintaining records, administering medications with supervision, transporting individuals to programs, and contributing to interdisciplinary team meetings to support treatment goals.

Qualifications

• H.S. Diploma or GED required; Associate degree preferred.
• Excellent oral & written communication skills; ability to learn computer systems.
• Experience in medical care or human services.
• Must pass AMAP certification training and maintain validity; CPR & First Aid.

Responsibilities

• Provide a safe, clean, home-like environment; inform Program Manager of needs.
• Inform supervisor of any serious incident or rights violations.
• Develop ADL and ILS related to residents.
• Maintain knowledge of residents' medical, psychological, cognitive and social needs for appropriate programming.
• Maintain and submit timely records and reports on residents' development and progress.
• Maintain and check medication administration records daily.
• Schedule residents' medication appointments and review medical records to ensure follow-up.
• Communicate with nurse and management about appointments and follow-ups.
• Participate in interdisciplinary treatment meetings and residents' reviews.
• Provide habilitation services per client service plans.
• Assist in transporting individuals to programs, appointments and activities.
• Assist clients with daily living, domestic living, recreation, and socialization.
• Report deficiencies or abuse; escalate safety issues to management.
• Ensure residents’ nutritional needs are met and assist with feeding.
• Organize, assist, supervise and prepare meals as needed.
