Qualifications

• High School Diploma or GED plus three years of relevant experience, or an Associate’s Degree or equivalent.
• Bachelor’s Degree preferred.
• MAP certification required within the first six months of employment.

Responsibilities

• Support the health, safety, dignity, and quality of life of individuals served.
• Coordinate medical appointments, follow‑up care, and team communication.
• Oversee medication administration and MAP documentation.
• Provide crisis intervention and emergency services, including on‑call support.
• Implement ISP goals and promote independence and community integration.
• Complete and review electronic health records, assessments, progress summaries, and required reports.
• Communicate with families, healthcare providers, and outside agencies.
• Monitor safety, fire drills, and facility conditions.
• Assist with benefits management including Social Security and housing subsidies.
• Supervise and train residential staff.
