Job summary

Qualifications

• HS diploma and minimum one year field-related experience required.
• Preferred health-care related AS or BA coursework or degree.
• Excellent written and verbal communication skills required.
• Ability to organize multiple priorities and stay calm under pressure.

Responsibilities

• Advocate for client caseload and ensure person-centered support.
• Develop, implement, and monitor treatment plans for clients.
• Oversee medication management for residents.
• Manage funds and related fiscal matters as needed.
• Serve as primary liaison for service providers, families, and guardians.
• Utilize an electronic health record system for client notes.
• Perform administrative/communication/reporting tasks and enter notes in Avatar.
• Report incidents to Site Manager while maintaining confidentiality.
