Behavioral Health Network, Inc (BHN) in Massachusetts is seeking a Care Coordinator II to join the Adult Community Clinical Supports (ACCS) team. You will oversee assessments, develop treatment plans, provide clinical interventions, and guide direct care staff to support individuals served.

The role requires a Master's degree in a mental health field and active clinical licensure (e.g., LCSW, LICSW, LMHC), plus at least two years in the human services sector.

Qualifications

• Master's Degree in a mental health-related field is required.
• Clinical licensure required (e.g., LCSW, LICSW, LMHC).
• Minimum of 2 years' experience in the human service field.
• Committed to empowerment and growth of individuals served and community integration.
• Knowledge of interface between mental health and medical conditions, substance use, care management, recovery principles, and trauma-informed treatment.
• Strong verbal and written communication skills.

Responsibilities

• Provide direct interventions with individuals served to complete comprehensive assessments, treatment plans, and clinical interventions.
• Provide modeling and clinical oversight for outreach and GLE direct care staff.
• Conduct and oversee clinical services set forth by the Integrated Team, performing functions reserved for LPHA as specified by contract.
• Oversee services provided to all individuals served by ACCS.
• Ensure the program adheres to clinical regulations and funding source requirements.
• Produce documentation that is clear, legible, and meets DMH and ACCS standards.
• Develop treatment plans consistent with evidence-based and best practices.
