Responsibilities

• Implement and monitor individualized care plans for residents.
• Ensure attendance at medical appointments, therapy, and support services.
• Advocate for residents by addressing concerns and preferences promptly.
• Maintain confidential records of care plans and incident reports.
• Ensure compliance with Arkansas state laws, Medicaid requirements, and facility policies.
• Assist in assessments to determine eligibility for services.
