Qualifications

• 3 years of experience working with Behavioral Health populations.
• Bachelor’s Degree required.
• Experience in a Scattered-site Supportive Housing setting.
• Working knowledge of AWARDS case management system.

Responsibilities

• Provide case management support to an assigned caseload of residents
• Develop and monitor service plans in coordination with residents and care teams
• Assist with referrals to medical, behavioral health, vocational, and community resources
• Document progress notes and maintain accurate case records in the agency system
• Support resident engagement through groups, events, home visits, and ongoing follow-up
