Qualifications

• Minimum of 2 years of experience working directly with individuals living with serious mental illness.
• Experience working within NYC communities and boroughs.
• Excellent verbal and written communication skills.
• Strong organizational and time management skills with the ability to work independently in a remote environment.
• Comfortable using technology and learning new software quickly.
• Willingness to become proficient in Google Workspace (Gmail, Calendar, Docs), Zoom, Slack, and other required technology platforms.
• High school diploma or equivalent.
• Driver’s license and willing to use personal vehicle (for Long Island only).

Responsibilities

• Conduct initial outreach to eligible members through phone, virtual, and in-person interactions.
• Engage and build trust: Locate and engage members in their communities, establishing strong, collaborative relationships with members and their care teams.
• Achieve enrollment goals: Meet daily enrollment and engagement targets to support program growth and member impact.
• Support recovery and self-determination: Partner with members to develop, track, and achieve personal recovery goals.
• Provide role modeling and encouragement: Serve as a consistent, positive presence throughout each member's recovery journey.
• Facilitate wellness and coping strategies: Encourage healthy behaviors and coping strategies that promote physical and behavioral health.
• Deliver physical health coaching: Provide physical health coaching in accordance with Vanna training and program guidelines.
• Drive continuous improvement: Share feedback with Vanna's technology and operations teams to improve workflows and the member experience.
• Maintain accurate and timely documentation while complying with HIPAA, health plan requirements, and Vanna Health policies.
