Star City Nursing and Rehabilitation is seeking an Activities Assistant to support the Activities Director in planning and directing a program that provides entertainment, exercise, relaxation, and expression for residents. You will assist in identifying needs and pursuing appropriate avocation and leisure activities, while supporting residents who are physically, emotionally, or cognitively impaired.

• Six months to one year of activity-related experience.
• Ability to lead in the absence of the Activity Director.
• Ability to work with residents who have cognitive, physical or sensory disabilities.
• Practical knowledge of how an Activity Department functions in a long-term care facility.
• General knowledge of regulatory requirements for an activity program in a long-term care facility.
• Outstanding interpersonal skills with high energy and enthusiasm.

Responsabilidades

• Identify improvement opportunities to enhance the resident experience.
• Assist in developing and implementing a program of activities to meet social, emotional, physical and other therapeutic needs of residents.
• Maintain detailed records of activity programs and participation of individual residents, identifying progress toward care plan goals.
• Participate in resident care planning by identifying activity needs in accordance with medical assessment.
• Prepare progress notes for medical staff reflecting residents’ reactions and progress.
• Maintain activity-related records required by regulations and Medical Records – activity assessments, progress notes, and discharge summaries.
• Assist in initiating and promoting activities both within and outside the facility when weather permits.
