Job summary

Plymouth Housing seeks a Case Manager to support residents on-site within the Resident Well-Being Care team in Seattle. You will deliver supportive services, promote housing stability, and work through harm reduction and trauma-informed practices across building programs.

The role is a designated caseload position, unionized with Local 8 of OPEIU, requiring collaboration with internal departments and community partners to ensure safety and well-being for residents.

Qualifications

• BA/BS in a human service field required.
• Minimum 1 year of experience as a case manager in housing or social services.
• Permanent Supportive Housing experience preferred.

Responsibilities

• Develop and maintain ISPs with residents to address housing stability and goals.
• Coordinate with community providers to ensure continuity of care.
• Assist residents with scheduling, transportation, and appointments.
• Respond to crises using trauma-informed, harm-reduction approaches.
• Maintain records and documentation with confidentiality and privacy standards.
• Collaborate with Housing Operations and internal teams on resident needs.
