Job summary

Plymouth Housing is hiring a Case Manager to support residents on-site in Seattle, WA, focusing on housing stability, harm reduction, and trauma-informed care. You will manage Individual Service Plans, coordinate with internal departments and community partners, and advocate for residents' needs with confidentiality and respect.

The role requires strong communication, organization, and the ability to respond to crises, while collaborating with the Residential Well-Being team to promote dignity

Qualifications

• BA/BS degree in a human service field; advanced education can substitute for experience.
• Minimum 1 year of case management experience in housing, behavioral health, or social services.
• Permanent Supportive Housing experience preferred.

Responsibilities

• Develop and maintain ISPs to address housing stability and goals.
• Assist with scheduling, transportation, and accompanying residents to appointments.
• Advocate and collaborate with community providers to ensure continuity of care.
• Coordinate with Housing Operations for stability tasks and paperwork.
• Respond to crises using trauma-informed, harm-reduction practices.
• Maintain records and documentation with strict confidentiality.
• Collaborate with internal teams and participate in team meetings.
