Job summary

Plymouth Housing in Seattle is seeking a Case Manager to support residents on the onsite Resident Well-Being Care team. This full-time unionized role advances housing stability through harm reduction and trauma-informed care.

The CM carries a designated caseload at one building, collaborates with internal departments, and maintains confidential documentation to support residents' well-being and independence.

Qualifications

• BA/BS in human services or related field required; advanced education can substitute for experience.
• 1 year minimum of case management experience in housing, behavioral health, or social services.

Responsibilities

• Develop and maintain ISPs with residents to address housing stability and goals.
• Assist residents with scheduling, transportation, and appointments.
• Advocate and collaborate with community providers to ensure continuity of care.
• Coordinate with Housing Operations for rental assistance and paperwork.
• Respond to crises using trauma-informed, harm-reduction practices.
