Job summary

Plymouth Housing in Seattle, WA is seeking a Case Manager to join the onsite Resident Well-Being Care team. This role provides supportive services to residents, promotes housing stability through harm reduction, trauma-informed care, and Housing First principles.

It carries a designated caseload at a given building and is unionized (OPEIU Local 8). The position offers a comprehensive benefits package for full-time employees, including Medical, Dental, Vision, PTO, 403(b), EAP, and subsidized

Qualifications

• BA/BS degree in a human service field is required; advanced education can substitute for experience.
• Minimum 1 year of experience as a case manager in housing, behavioral health, or a social service setting.
• Experience in Permanent Supportive Housing is preferred.

Responsibilities

• Develop and maintain ISPs to address housing stability and residents’ goals.
• Know resources in Plymouth and the community to maximize resident access.
• Assist residents with scheduling, transportation, and appointments.
• Advocate and collaborate with community providers to ensure continuity of care.
• Coordinate with Housing Operations for rental assistance and paperwork.
• Respond to crises using trauma-informed, harm reduction practices.
• Address complaints and escalate to the Residential Well-Being Manager as needed.
• Collaborate with internal behavioral health team for cohesive service delivery.
• Attend supervision and team meetings to discuss resident issues and program goals.
• Work with Plymouth departments for information collection and program activities.
