Job summary

Plymouth Housing is seeking a full-time Case Manager to support residents with individualized service plans, scheduling, and access to community resources. The role emphasizes harm-reduction and trauma-informed care, with a designated caseload at a specific building.

The CM works on a unionized team, coordinating with internal departments to ensure housing stability and resident wellbeing, while maintaining confidential documentation and client-centered services.

Qualifications

• BA/BS degree in a human service field.
• Minimum 1 year of experience as a case manager in housing, behavioral health, or a social service setting.
• Experience in Permanent Supportive Housing is preferred.

Responsibilities

• Develop and maintain ISPs for residents addressing housing stability and goals.
• Assist residents with scheduling and transportation to appointments.
• Advocate and collaborate with community providers for continuity of care.
• Coordinate with Housing Operations to support stability and paperwork.
• Respond to crises using trauma-informed, harm-reduction practices.
• Maintain accurate records and documentation with confidentiality.
