Job summary

Plymouth Housing is seeking a Case Manager to support residents on-site within the Resident Well-Being Care team. You will provide preventive and responsive services, maintain strong documentation, and promote housing stability through harm reduction and Housing First principles.

You will manage a designated building caseload, coordinate with housing operations and community partners, and participate in team supervision to align with Plymouth's mission and values.

Qualifications

• BA/BS degree in a human service field.
• Minimum 1 year of experience as a case manager in housing, behavioral health, or social services.
• Experience in Permanent Supportive Housing is preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Develop and maintain ISPs with residents to address housing stability and goals.
• Maintain knowledge of resources to maximize resident access.
• Assist residents with scheduling, transportation, and appointments.
• Advocate and collaborate with community providers for continuity of care.
• Coordinate with Housing Operations to support stability and paperwork.
• Respond to crises using trauma-informed, harm-reduction practices.
• Maintain records and documentation with confidentiality.
• Attend supervision and team meetings to discuss resident issues and goals.
• Work with internal teams to support resident services and building operations.
