Responsibilities include planning driving schedules, transporting residents to inpatient/outpatient programs, reporting changes in condition, handling medical records transport, and maintaining vehicle safety and cleanliness, while ensuring resident hydration and safety.

Qualifications

• Valid Driver's Licensure for the state you're applying in.
• State Approved Nurse Aide Training and Competency Evaluation Program; Nurse Aide on State Registry in good standing.
• Current BLS/CPR certification as required by state.

Responsibilities

• Organizes transportation of residents to required appointments based on priority of resident needs.
• Transports residents to inpatient and outpatient programs and to facilities and residences.
• Develops driving schedule based on appointment needs; transports residents after hospital discharge.
• Reports changes in resident condition to clinical supervisor and maintains safety during transport.
• Transports medical records for signatures to medical offices.
• Keeps van clean and safely maintained; logs mileage and ensures routine vehicle maintenance.
• Assists staff with safe lifting/transferring per physician orders and resident needs.
• Establishes contact with outside facilities to coordinate services and scheduling.
