Job summary

Aztec Healthcare in Aztec, NM seeks a Transportation Coordinator to organize resident transportation to appointments, coordinate with clinical staff, and ensure timely, safe patient transfers. The role requires a valid driver’s license and current BLS/CPR certification.

The coordinator will schedule trips, maintain vehicle safety and cleanliness, and communicate with hospitals and clinics to arrange services. This full-time on-site position offers benefits and growth opportunities.

Qualifications

• Valid driver’s license for the state in which you are applying.
• Current BLS/CPR certification (as required by state)

Responsibilities

• Organizes transportation of residents to complete required appointments based on priority of resident needs.
• Transports resident to and from inpatient and outpatient programs and to and from appropriate facilities and residences.
• Develops driving schedule based on resident’s scheduled medical appointments and program destination.
• Provides report and assessment of resident’s behavior or changes during transportation and reports changes to clinical staff.
• Transports medical records for signatures to medical offices.
• Maintains safe operation of the van and vehicle cleanliness and routine maintenance logs.
• Assist clinical staff with safe lifting, transferring, and transporting as directed.
• Establishes positive communications with outside facilities and hospitals to ensure proper services are scheduled.
• Reports scheduling changes to DON and nursing/ancillary staff and follows up as needed.
• Provide adequate hydration to residents daily according to their needs.
• Protect residents’ personal property and ensure safety policies are followed.
• Other duties may be assigned as needed.
