Job summary

Aztec Healthcare in Aztec, NM, is seeking a Transportation Coordinator to coordinate patient transportation to appointments and discharge-related transport. You will manage schedules, ensure safety, and support clinical staff during transfers.

Responsibilities include organizing transport, maintaining vehicles, and communicating with hospitals and clinics to ensure timely services for residents.

Qualifications

• Valid Driver's Licensure for state in which you are applying.
• Current BLS/CPR certification (All SNFs; ALFs as required by state)

Responsibilities

• Organizes transportation of residents to complete required appointments based on priority of resident needs.
• Transports resident to and from inpatient and outpatient programs and to and from appropriate facilities and residences.
• Develops driving schedule based on resident’s scheduled medical appointments and program destination.
• Provides transportation of current or new residents from hospital after discharge.
• Reports changes of abnormal findings in resident status during transport to the clinical supervisor.
• Transports medical records for signatures to medical offices.
• Maintains van in a neat and clean condition and ensures routine maintenance.
• Assists clinical staff with safe lifting, transferring, and transporting as specified by orders and clinical supervisor.
• Establishes communication with outside facilities and physicians to ensure proper services are scheduled.
• Reports scheduling changes to DON and nursing staff and follows up as necessary.
• Provides adequate hydration to residents at regular intervals as per resident needs.
• Protects residents' personal property and maintains safety during transport.
• Other duties may be assigned as needed.
