Qualifications

• Certified Nurse Aide in the state of practice is required.
• Completion of State Approved Nurse Aide Training and Competency Evaluation Program; name must appear on State Registry as Nurse Aide in good standing.
• Current BLS/CPR certification is required for all SNFs; ALFs as required by state.

Responsibilities

• Coordinate transportation of residents to appointments based on priority of needs.
• Transport residents to inpatient and outpatient programs and to/from facilities and residences.
• Develop driving schedules around residents' medical appointments and program destinations.
• Observe and report changes in resident condition during and after transport to clinical staff.
