Qualifications

• Certified Nurse Aide in good standing with the state registry.
• Successful completion of State Nurse Aide Training and Competency Evaluation Program.
• Current BLS/CPR certification required for all SNFs/ALFs as applicable.

Responsibilities

• Organizes transportation of residents to appointments based on priority of needs.
• Transports residents to inpatient/outpatient programs and facilities.
• Maintains safety and vehicle cleanliness during transport.
• Assists with safe lifting and transfers as directed by clinical staff.
• Reports changes in resident condition to supervisor after transport.
• Maintains communication with outside facilities to coordinate services.
