Qualifications

• Bachelor’s degree in a Human Service field required; two years' residential case management or counseling experience.
• Associate degree with related experience (5+ years) suitable for Residential Case Manager I.

Responsibilities

• Provide support services to residents on an assigned caseload.
• Develop and document individual service plans to support rehabilitation goals.
• Maintain accurate case records and progress notes for each resident.
• Offer instruction and service delivery across 11 service categories through guidance and support.
• Manage crises within the residence and document/events per policy.
• Assist with medication management per Licensed Housing Policy and Procedures.
• Provide counseling to clients/residents as required.
• Lead recreational and counseling groups and escort residents to appointments.
• Coordinate community activities and document significant events in logs.
• Adhere to PCMH policies and procedures and participate in staff meetings.
