Postgraduate Center for Mental Health seeks a direct service clinician to join the ACT Team in New York. This role requires a Master’s degree with NY licensure and experience in crisis intervention and community-based services.

The clinician will perform community visits, coordinate care, facilitate harm reduction groups, and engage families while collaborating with team members to support integrated dual disorder treatment.

Qualifications

  • Master’s Degree required - LMHC, LMSW, LCSW, or equivalent licensure in NYS.
  • At least one year of experience working with individuals with serious mental illness with co-occurring substance use disorders.

Responsibilities

  • Conducts daily community visits and completes progress notes per OMH standards.
  • Identifies treatment services and objectives for the consumer’s treatment plan.
  • Facilitates weekly Harm Reduction and Relapse Prevention groups.
  • Participates in daily meetings and informs ACT team of substance use issues.
  • Involves family/significant others in treatment and provides relapse prevention education.
  • Provides 24-hour crisis intervention on-call services on rotating basis.
  • Performs other duties aligned with program goals as directed by Team Leader.
