Job summary

The Arboretum of Winnie in Texas is seeking a dedicated Social Worker to support residents’ social and emotional well-being. You will develop care plans, assess needs, coordinate discharge planning, and work with families and staff to ensure compassionate, holistic care.

The ideal candidate holds a BSW or Social Worker Designee, has strong communication, and prior long-term care experience. A collaborative team, ongoing training, and community partnerships are core to our approach.

Qualifications

• Requires a Bachelor's Degree in Social Work (BSW) or Social Worker Designee.
• Demonstrates effective communication and interpersonal skills.
• Experience in long-term care preferred.

Responsibilities

• Develop detailed social histories and assessments for residents.
• Create individualized care plans supporting resident goals and well-being.
• Participate in interdisciplinary care plan conferences.
• Provide individual and group support to residents and families during crises.
• Assist with new resident orientation and transitions.
• Coordinate discharge planning for smooth transitions.
• Facilitate staff in-service training on social services.
• Build partnerships with hospitals, doctors, churches and community agencies.
