Job summary

Blue Springs Wellness & Rehabilitation is seeking a compassionate and driven Social Services Designee to support residents' emotional, social, and psychological well-being in our skilled nursing setting.

You will conduct psychosocial assessments, assist with transitions to long-term and memory care, complete MDS-related documentation, advocate for resident rights, and collaborate with an interdisciplinary team to ensure survey readiness and regulatory compliance.

Qualifications

• Certified, licensed, or registered in the state of practice (required).
• Minimum of 1 year of social work experience in a healthcare or geriatric setting.
• SNF experience preferred; nursing experience in a SNF is a plus.

Responsibilities

• Conduct psychosocial assessments and contribute to individualized care plans.
• Support residents and families through transitions into long-term care and memory care.
• Complete MDS-related documentation accurately and in compliance with regulations.
• Advocate for resident rights, dignity, and self-determination.
• Coordinate discharge planning and connect residents with community resources.
• Collaborate with nursing, administration, and the interdisciplinary team.
• Assist with survey readiness and regulatory compliance efforts.

Skills

Eligible for licensure in state

1+ year social work experience

SNF experience preferred

Nursing experience in SNF is a plus

Education

Bachelor’s degree in Social Work or related Human Services field
