Job summary

PACS Group seeks a dedicated social services professional to plan, develop, organize, implement, and evaluate our facility's social service programs to meet residents' medically related and psychosocial needs.

The role requires collaboration with administration and medical staff, development of policies, and maintaining progress notes. A health-care background and relevant education are preferred to support residents and families effectively.

Qualifications

  • Must possess a high school diploma or equivalent; Associate or Bachelor's degree preferred but not required.
  • Prior health-care experience preferred.
  • Ability to read, analyze, and interpret general business materials.
  • Ability to write reports and procedure manuals.
  • Ability to effectively present information to managers and staff.
  • Ability to apply fractions, percentages, ratios and proportions.
  • Ability to solve practical problems with concrete variables; follow written and oral instructions.

Responsibilities

  • Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
  • Meet with administration and staff to plan social services.
  • Develop, administer, and coordinate department policies and procedures.
  • Maintain social service progress notes and coordinate with other departments.
  • Provide information on Medicare/Medicaid and financial assistance.
