Job summary

PACS Group seeks a Social Services professional to plan, develop, implement, evaluate, and direct our facility's social service programs in line with federal, state, and local standards.

You will coordinate with administration, medical and nursing staff, and community partners to ensure residents receive comprehensive psychosocial support and resources. This role emphasizes confidential documentation, discharge planning, and helping residents achieve optimal self-care and well-being.

Qualifications

• Must have a high school diploma; associate or bachelor's degree preferred.
• Experience in health-care settings is preferred.
• Ability to read, analyze, and interpret regulations and procedures.

Responsibilities

• Assist in planning and directing social service programs.
• Meet with administrators and staff to plan social services.
• Develop and coordinate department policies and procedures.
• Maintain social service records and progress notes.
• Provide residents with information on Medicare/Medicaid and financial assistance.
• Discharge planning and resource coordination.
