Resident Social Services Advocate

Job summary

PACS is seeking a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in alignment with federal, state, and local standards.

You will collaborate with administration, medical staff, and other departments; develop policies; maintain progress notes; and guide families on Medicare/Medicaid options to support residents’ well-being.

Qualifications

• Must possess, at a minimum, a high school diploma or its equivalent; Associate or Bachelor’s Degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff, and other related departments in planning social services.
• Assist in developing and coordinating department policies and procedures.
• Record and maintain regular Social Service progress notes and coordinate with other departments as needed.
• Provide information to residents and families about Medicare/Medicaid and financial assistance programs.
