Atrium Centers is seeking a Social Services Assistant to support the Social Services Director in implementing programs and documenting residents’ needs in a 100% employee-owned organization. You’ll collaborate with residents, families, and interdisciplinary teams to ensure dignified care and compliance with guidelines.

The role emphasizes documentation, care planning, and coordination to support residents’ social and emotional well-being within a skilled nursing setting.

Qualifications

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree).
• Experience in a skilled nursing or long-term care setting preferred.
• Strong written and verbal communication skills, with ability to interact professionally with residents, families, and teams.
• Ability to assist with documentation, assessments, care planning, and regulatory requirements under supervision.

Responsibilities

• Assist the Social Services Director in implementing and carrying out social service programs in accordance with state and federal guidelines.
• Support the identification and documentation of residents’ social, emotional, and psychological needs.
• Provide service coordination and support for residents and families, including communication and follow-up.
• Participate in care planning and interdisciplinary team meetings as assigned.
• Maintain accurate, timely documentation to support compliance and regulatory standards.
