Job summary

Stoney Point Healthcare Center is seeking a Social Services Coordinator to assist in planning, developing, and directing social service programs for residents. You will collaborate with administration, medical and nursing staff to ensure the highest level of resident well-being and support services.

The role requires strong reading, writing, and communication skills, plus the ability to analyze policies and maintain confidential resident information.

Qualifications

• Must possess a high school diploma or equivalent; Associate or Bachelor’s Degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business materials.
• Ability to write reports and present information to managers and staff.
• Strong organizational and budgeting skills.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review policies annually and participate in changes.
• Coordinate social service activities with other departments.
• Assist with discharge planning and arranging ancillary services.
• Maintain confidentiality of resident information and protect protected health information.
