Job summary

Rockport Healthcare Services is seeking a Social Service Coordinator to address residents’ psychosocial and concrete needs within regulatory frameworks. You will orient new residents, plan discharges, update care plans, and manage referrals.

The role requires collaboration with families and staff, coordination of financial affairs, and active participation in QMPI activities to improve performance and resident wellbeing.

Qualifications

• Bachelor's degree in social work or human services required.
• LSW required in TX Centers; one year supervised health care experience.
• High School Diploma or equivalent for centers under 120 beds.
• Strong written and verbal communication and interpersonal skills.
• Knowledge of social resources in the community.
• Healthcare setting experience preferred.

Responsibilities

• Identify and meet residents' psychosocial and concrete needs per regulations.
• Orient residents to the center.
• Discharge planning.
• Update Resident Care Plan and Social History.
• Maintain records of outside referrals.
• Assist with coordination of residents' financial affairs.
• Communicate needs and plan of care to residents, families and staff.
• Participate in meetings, in-services and training as required.
• Organize and facilitate Family Council.
• Implement QMPI quality measurement and performance improvement program.
• Assist with coordination of resident room moves.
• Present professional image to consumers and protect resident rights.
• Organize and maintain a professional work setting.
