Job summary

Stoney Point Healthcare Center is seeking a Social Services professional to plan, implement, and direct our social service programs in compliance with federal, state, and local standards. You will work with administration and clinical staff to ensure holistic resident care and coordinated community resources.

The role involves developing policies, maintaining progress notes, and guiding staff orientation to address residents' social, emotional, and financial needs.

Qualifications

• Ability to read, analyze, and interpret general business materials, journals, and regulations.
• Strong written communication for reports and policies.
• Effective oral presentational skills to managers and staff.
• Interpersonal skills to coordinate with multiple departments and families.

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop and coordinate department policies and procedures.
• Review policies annually and suggest changes.
• Provide information on Medicare/Medicaid and financial assistance programs.
• Record and maintain social service progress notes and treatment responses.
• Coordinate activities with other departments and community agencies.
• Assist residents in achieving independence and wellbeing.
• Develop and participate in staff orientation and training related to social needs.
