Job summary

Stoney Point HealthCare Center is seeking a Social Services Coordinator to plan, develop, and direct our facility’s social service programs in line with federal, state, and local standards. Responsibilities

• Assist the Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services as directed.
• Develop and coordinate department policies and procedures; review annually.
• Identify medically related social and emotional needs of residents and coordinate services.
• Maintain regular social service progress notes and discharge planning with community resources.
