Job summary

PACS is seeking a Social Services Coordinator to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in line with federal, state, and local standards.

You will work with administration, medical staff, and other departments to ensure residents receive comprehensive social services, coordinate community resources, and maintain documentation to support resident well-being and program effectiveness.

Qualifications

• Must possess at minimum a high school diploma or its equivalent. An Associate or Bachelor’s Degree is preferred but not required.
• Ability to read, analyze, and interpret general business materials and regulations.
• Ability to write reports, business correspondence, and procedure manuals.

Responsibilities

• Assist Social Service Director in planning, developing and directing social service programs.
• Meet with administration and staff to plan services and policies.
• Develop and coordinate department policies and procedures; review annually.
• Coordinate discharge planning and external resources for residents.
• Provide information about Medicare/Medicaid and other financial assistance options.
• Document progress notes and coordinate with other departments to meet resident needs.
