Job summary

Sullivan Park Care Center is seeking a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs.

The role involves collaborating with administration, medical staff, and departments to ensure the residents' medically related social and emotional needs are met, while upholding confidentiality and compliant practices.

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent.
• Associate or Bachelor's Degree preferred but not required.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff and other departments to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review policies at least annually and propose changes.
• Assist residents in achieving the highest practicable level of self-care and well-being.
• Coordinate social service activities with other departments as necessary.
• Maintain confidentiality of resident care information.

Education
