Job summary

Sullivan Park Care Center in Spokane Valley, WA is seeking a Social Service professional to plan, develop, and direct our facility's social service program, ensuring the emotional, social, and medical needs of residents are met with compassion.

You will collaborate with administration and health care staff, develop policies, assist in discharge planning, provide information on Medicare/Medicaid, and maintain confidential records while promoting residents’ independence and quality of life.

Qualifications

• Must have a high school diploma or equivalent; higher degree preferred but not required.
• Prior health-care experience preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration and nursing staff to plan social services.
• Assist in developing and coordinating department policies and procedures.
• Discharge planning and coordinating with community agencies.
• Provide information to residents/families about Medicare/Medicaid and other financial programs.
• Maintain confidentiality of resident information and records.
