Job summary

PACS seeks a social services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs. You will work with administration and medical staff to plan services and ensure residents' medically related social needs are met, in line with federal, state, and local standards.

Qualifications

• Must have a high school diploma or equivalent; an Associate or Bachelor's degree is preferred but not required.
• Prior health-care experience is preferred.
• Ability to read, analyze, and interpret general business materials and regulations.

Responsibilities

• Assist in planning, developing, and directing social service programs.
• Meet with administration and staff to plan social services.
• Assist in developing and coordinating department policies and procedures.
• Review policies annually and suggest changes.
• Provide information to residents/families about Medicare/Medicaid and other aid programs.
• Record social service progress notes and share with treatment teams.
• Coordinate social services with other departments and external agencies.
