Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent; degree preferred but not required.
• Ability to read, analyze, and interpret business materials, journals, procedures, or regulations.
• Ability to write reports, correspondence, and procedure manuals.
• Ability to present information and respond to questions from managers and employees.
• Ability to apply fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems with limited standardization.
• Ability to interpret instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review policies annually and participate in changes.
• Identify medical-related social and emotional needs of residents.
• Coordinate social service activities with other departments as needed.
• Assist residents to attain highest practicable level of self-care and well-being.
• Provide information on Medicare/Medicaid and financial assistance programs.
• Maintain social service progress notes and reports.
• Assist in discharge planning with community agencies.
