Qualifications

• High school diploma required.
• Prior experience in social services or long-term care preferred.
• Knowledge of resident rights and psychosocial needs of the elderly preferred.

Responsibilities

• Identify residents' psychosocial and emotional needs.
• Complete social histories and assessments.
• Participate in care planning and resident conferences.
• Support resident rights and grievance resolution.
• Assist with PASRR, advance directives, and education on rights.
• Support admissions, room changes, and transitions.
• Coordinate discharge planning and community referrals.
• Assist with Medicaid applications and benefit coordination.
• Document progress notes and interactions timely.
• Help with surveys, audits, and regulatory compliance.
• Participate in quality improvement activities.
• Maintain confidentiality and HIPAA compliance.
