• Assist residents and families with Medicare/Medicaid and financial options.

Responsibilities

• Help plan and direct social services in the facility.
• Coordinate with departments to ensure resident needs are met.
• Assist in discharge planning and referrals to community resources.
• Prepare required forms and reports for the Social Service Director.
• Provide information and consultation to staff and families on services.
